The Stswecem’c Xgat’tem First Nation is seeking an enthusiastic, highly motivated individual to fill the position of the Community Health Representative/Injury Surveillance Clerk.

The Community Health Representative/Injury Surveillance Clerk contributes to the overall success of the communities by providing health care services to community members requiring assistance, such as new mothers or recently released patients from medical facilities. The CHR/IS Clerk works closely with other health care workers to provide education, case management, preventative care, grief management and other services to assist clients with their health concerns. The position also involves the collection, organization, preparation and entry of data of injury surveillance forms.

Skills and Abilities:

  • Ability to assist individuals, families and health team on health matters, make appropriate referrals and promote healthy living options
  • Ability to participate in case management with health team members regarding needs of individuals and families
  • Ability to conduct home visits, support young parents and families and provide information and training on accessing health care to a spectrum of community members
  • Ability to collect, organize, prepare and enter injury surveillance data into Microsoft Excel (training will be provided)
  • Must have strong organizational and communication skills and a passion for working in the community

Education and Experience:

  • Diploma in Practical Nursing, Health or Social Work
  • Two to three years related experience

Working Conditions

  • Travel will be required
  • Some non-standard hours of work
  • Receives minimal supervision with occasional direction and few checks of the work performed

Conditions of Employment

  • Must be able to obtain and maintain a Criminal Records Check
  • Must have a valid BC Driver’s License
  • Class 4 Drivers Licence would be an asset
  • Must provide a vehicle in good operating condition and appropriate vehicle insurance to meet program
